The hypothalamus is a crucial part of the brain, located at the anterior end of the diencephalon, below the hypothalamic sulcus, and in front of the interpeduncular nuclei. It is divided into various nuclei and nuclear areas, controlling most of the body’s vegetative and endocrine functions, as well as many aspects of emotional behavior.
Anatomical Structure
The hypothalamus extends from the optic chiasma to the mammillary body and is divided into four main regions:

Mammillary Region
- Contains nuclei such as the mammillary bodies and the posterior hypothalamus nucleus.

Tuberal Region
- Includes the dorsomedial nucleus, ventromedial nucleus, and arcuate nucleus.
- Ventral Surface Structures:
- Tuber Cinereum: An elevated mass of grey matter forming part of the median eminence. Function: Contains neurons that secrete hypothalamic regulating hormones.
- Infundibulum: A stalk-like structure attaching the pituitary gland to the hypothalamus. It lies superior to the optic chiasm and includes nuclei such as:
- Paraventricular nucleus
- Supraoptic nucleus
- Anterior hypothalamus nucleus
- Suprachiasmatic nucleus
- The suprachiasmatic nucleus is considered part of the hypothalamus as it regulates certain autonomic activities.

Hormonal Functions
Secretion of Posterior Pituitary Hormones:
- The hypothalamus secretes posterior pituitary hormones:
- Antidiuretic Hormone (ADH): Secreted by the supraoptic nucleus.
- Oxytocin: Secreted by the paraventricular nucleus.
- These hormones are transported via axoplasmic flow through the fibers of hypothalamohypophyseal tracts to the posterior pituitary.
Control of Anterior Pituitary:
- The hypothalamus regulates the anterior pituitary by secreting releasing and inhibitory hormones:
- Growth Hormone-Releasing Hormone (GHRH)
- Growth Hormone-Releasing Polypeptide (GHRP)
- Growth Hormone-Inhibiting Hormone (GHIH) or somatostatin
- Thyrotropin-Releasing Hormone (TRH)
- Corticotropin-Releasing Hormone (CRH)
- Gonadotropin-Releasing Hormone (GnRH)
- Prolactin-Inhibiting Hormone (PIH)
- These hormones are secreted by discrete areas of the hypothalamus and transported to the anterior pituitary via the hypothalamohypophyseal portal blood vessels.
Regulation of Adrenal Glands

Adrenal Cortex:
- Controlled by the anterior pituitary secreting Adrenocorticotropic Hormone (ACTH).
- ACTH secretion is regulated by CRH from the paraventricular nucleus.

Adrenal Medulla:
- The dorsomedial and posterior hypothalamic nuclei respond to emotional stimuli.
- They send impulses to the adrenal medulla through sympathetic fibers, triggering the release of catecholamines in response to emotional stress.
Autonomic Nervous System (ANS) Regulation

- Regulated by the posterior and lateral nuclei of the hypothalamus.

- Regulated by two mechanisms:
- Thirst Mechanism: In the lateral nucleus initiates the sensation of thirst due to increased ECF osmolality.
- ADH Mechanism: Stimulation of the supraoptic nucleus causes ADH release, leading to renal water retention when ECF volume decreases.

Sleep and Wakefulness:
- Mammillary Body: Acts as a wakefulness center; stimulation leads to wakefulness, while lesions lead to sleep.
- Anterior Hypothalamus: Stimulation leads to sleep.
Behavioral and Emotional Changes
- Responding Systems:
- Reward Center (ventromedial nucleus and medial forebrain bundle): Electrical stimulation induces pleasure and satisfaction.
- Punishment Center (posterior and lateral nuclei): Electrical stimulation induces pain, fear, defense, escape reactions, and other punishment-related responses.
Sexual Function and Sensory Response

Sexual Function:
- Regulated by the secretion of GnRH.
- Arcuate and posterior hypothalamic nuclei are also involved.

Response to Smell:
- The posterior hypothalamus, along with the hippocampus and brainstem nuclei, is responsible for autonomic responses to olfactory stimuli.

Circadian Rhythm:
- Developed in response to recurring daylight and darkness.
- The Suprachiasmatic Nucleus sets the biological clock through connections with the retina via retinohypothalamic fibers.

- Characterized by moon face, obesity, polydactyly, mental retardation, and hypogenitalism.
